How You Can Help Us to Serve You Better



Serving Patients

Since all our JOI- Beaches physicians are surgeons, their surgery schedules dictate our office schedules. In fact, the majority of their time is spent in surgery; leaving about 20 hours per week for office visits. As a result, there is a great need for flexibility and understanding by our patients. We appreciate yours. It is our policy that the same physician will care for you throughout the course of your treatment, in order that you will receive consistency and continuity of care. To help him be more efficient, each physician has a medical assistant to handle your medical issues and prescription requests. Here are some ways you can help us serve you better:

Appointments (to help avoid delays)
A. We provide all paperwork online, via fax, mail so you can complete your paperwork at home ahead of time.
B. Please take the time to fully and legibly complete all paperwork we require and bring it with you. Use black ink.
B. Please arrive 15 minutes prior to your scheduled appointment time so we can process all paperwork.
C. Bring your driver's license and all relevant insurance cards for us to copy so we can bill your insurance.
D. If you have had x-rays, lab reports, MRI's, or doctors' notes prepared elsewhere, bring them with you, as these are a critical part of the doctor's evaluation and treatment with you.

Referrals: For insurances requiring a referral (all HMOs and some POSs) please obtain a referral from your primary care physician PRIOR TO YOUR SCHEDULED APPOINTMENT. The referral is authorization by your insurance company for our physician to treat you. If you are unable to obtain the referral before your scheduled appointment, we will gladly reschedule your appointment to allow time to obtain your referral.

Co-Pays, Dedutibles and Self-Pay Patients: Self-pay patients are asked to make full payment at the time of your appointment. All co pays and unmet deductibles are also collected at the time of service.

Medical Records Requests: You are required by law to sign a release to obtain your medical records. You may pick the release up at our office, or we can fax or mail the form to you. There is a processing fee of $10.00 cash prepay for each x-ray film to cover supply costs.Processing your request takes 7 - 10 business days.

Completion of Forms: Many patients have disability and gap insurance to provide them supplementary income during their convalescence period. These companies require our doctors to fill in their forms. Patients need to fill in the Patient Section of these forms and bring them with you to your appointment. Our office will complete the physician section. There is a $10.00 CASH prepay fee per form to cover the time expended to process your paperwork. Please allow 7 - 10 business days for completion. We will gladly fax the forms on your behalf.

Voicemail Messages: Please do not be offended if your call is transferred to voicemail. When the medical assistants and staff are with a physician or patients, they cannot answer the phone in person. Please clearly state your first and last name, date of birth, a phone number where you can be reached and a description of your issue.

Prescrition Requests: Please call our office (not your pharmacy) at least 24 hours before you run out of your medication. Leave your first and last name, date of birth, phone number of your pharmacy, and the medicine you request. Requests can take up to 24-48 hours to be processed with your pharmacy. Always check with your pharmacy after 6:30 p.m. before re-phoning our office. WE DO NOT FILL PRESCRIPTIONS AFTER HOURS OR ON WEEKENDS. Prescriptions requested after noon on Friday will not be processed until the following business day.

Office Delays: From time to time, based on the nature of Orthopaedic injuries, immediate care and procedures need to be performed in the office, which may cause unexpected delays on the part of our office staff and physicians. Please bear with us when these times arise. We announce these delays, and give you the opportunity to reschedule your appointment. We apologize for any inconvenience this may cause you. We can assure you it is not intentional, and make every effort to keep these situations to a minimum.

After Hours: Calls made after hours are for urgent matters only. For routine matters we ask that you call during normal business hours.

We accept a wide variety of plans. Please call our office at (904) 241-1204 to verify the plans we participate.

Pre-surgical Preparation
Pre-surgical and pre-procedural formalities and instructions are disseminated and reviewed extensively in the office, tailoring to individual patients needs.

Postoperative Information
All postoperative instructions are discussed, and written instructions are provided to the patient after the surgical procedure has been performed.

Emergency / After Hours Procedures
For post procedural or postoperative problems please call the after hours number provided. For other urgent/emergent conditions that are life or limb threatening, go to the nearest emergency facility. For new patients not seen before at JOI, please contact the nearest emergency room or emergency clinic. We will be happy to follow-up with you after that initial contact.
